SUMMARY

RESPONSIBILITIES
· Actively participate in planning and training activities to promote comprehensive and integrated reproductive and sexual health services within the health center. Work cooperatively with Medical Director, Clinical Operations Director, QI Manager and others.
· Ensure implementation of objectives and activities as outlined in the FPSP work plan.
· With QI Manager, ensure that clinic protocols for medical and nursing care are in accordance with the MN Family Planning Rules.
· Coordinate with clinical nursing and other staff to design and implement outreach/in-reach and education activities focusing on family planning for clinic patients and community members.
· Increase community clinic expertise in working with targeted populations through in-service training and related activities.
· Provide comprehensive, non-directive family planning counseling for patients.
· Train and supervise health educator/outreach staff funded under family planning grant.
· Develop tools for public information and outreach and ensure implementation of outreach work plan, with Peer Educator.
· Actively participate in staff meetings and other designated agency meetings.
· Participate in external meetings and task forces to build effective relationships, share information and network with funders, relevant community organizations and coalitions.
· Develop and conduct group educational sessions for parents/caregivers on talking to children about sexual behavior. Develop and implement other group education activities and focus groups.
· Manage, track and report service delivery and outcomes to meet agency and funder needs and requirements, including budget management, narrative and statistical reporting. Assist with related audits and evaluation activities, as requested by QI Manager.
QUALIFICATIONS
· BS in Public Health, Nursing or related discipline, with at least two years’ clinic-based experience in family planning/sexual and reproductive health—and other areas such as health care access, sexuality. For RN, graduation from an accredited school of professional nursing and current RN license in
Minnesota
.
· Experience, competence and sensitivity to working with diverse ethnic/racial populations, new immigrants and low-income individuals.
· Demonstrated ability to communicate with and work effectively with a variety of constituents including clinical and administrative staff, funders, community members, patients, and others.
· Health education counseling experience.
· Small group facilitation, curriculum development and program evaluation skills.
· Supervisory experience with outreach or community-based staff.
· Excellent problem-solving, oral and written communication and record-keeping skills.
· Ability to prioritize, organize and carry out work assignments as assigned.
· Proficient in use of computer programs including Word, Excel and PowerPoint. Ability to extract data from practice management system using standard ‘reports.’
· Familiarity with North and North Minneapolis communities preferred.
